Point slope form y-y1=m(x-x1)

where m is the slope =( y2-y1)/(x2-x1)

Example 1: if slope is given as 4 & point (-3,6) then point slope form is 

y-6=4(x-(-6))

y-6=4(x+6)

Example 2: if two points given (3,4) (5,6)

slope (m)=(6-4)/(5-3)=1

y-4=1(x-3)
