Child day care center operators can no longer use written form 2971 (Request for a Criminal History and Central Registry Check) to submit requests unless they receive permission from their Licensing representative. To request permission to submit background checks in this manner, the operation must first submit a Form 2945 (Child Day Care Center Request to not submit Background Check Online Form) to their Licensing representative, who will review the form and determine approval or denial of the request. The Request to Not Submit Background Checks Online must be time-limited and the provider must indicate the date they plan to start submitting their background checks online.
