No. The Communities Transformation Grant awards are new and authorized under the Affordable Care Act of 2010, they are not a replacement for any existing federal program. The Community Transformation Grant initiative provides an important opportunity for communities to support intensive approaches to reduce risk factors responsible for the leading causes of death and disability and to prevent and control chronic diseases in the nation.
