No specific conditions, they may have external male organ and internal female organs, or vise verse. Or even one organ prominent, with another underdeveloped. Except perfect sex organs, both external and internal in men and women, all other abnormality people are called ” Eunuchs “. To be precise, your question itself is wrong, a Hermaphrodites means, an animal or plant, which have both male and female sex organs, and produce off springs. Most of the flowers are hermaphrodites.

True hermaphrodites (complete male and female reproductive systems and genitalia) don’t exist in the human species. There are plenty of true hermaphrodites in the animal kingdom (some mollusks and worms are the first to come to mind), but not humans. Intersex individuals do exist in the human species, they tend to have EITHER functioning ovaries, functioning testes, or neither, and also can have different forms of genitalia, some similar to a penis, some to a vagina, some intermediate.

People that have been called hermpahrodite in the past are not actually true hermaphrodites, they are instead females with long clitorus or male genetalia that is not fused and appears female….. Some woman can be born with 2 uterui (hmmm not sure of ending on that one) but I’m not sure if someone can have male and female working genetalia as such….one would most likely be non-functioning or “vestigial”.
