• Use EUCLID to find the correct citation for the item. • Click the oval REQUEST button and choose the following option – “Music & Media Reserve – Video, Audio, Books, Scores” . • Fill out the request form and click on PLACE REQUEST. • Specify Titles and Track Numbers. In person: • Fill out GREEN FORM (printable form). • Be sure to include the following- Instructor Name, Course Name, Course Number and Section Number (including all cross listings). Recording Name. Recording Number. • Specify Titles and Track Numbers. Preference will be given to selections contained in materials in our collection. Personal copies of recordings acceptable if substitutes are unavailable. No second generation copies of recordings are acceptable.
