If you have a credit card and want to order your birth certificate on the telephone, you may use this toll free number: 1-877-817-7362 NOTE: Upon request, the Office of Vital Records also provides copies of death certificates, marriage certificates and divorce certificates. If your request is in writing, there are specific guidelines you must follow in your request. You may download forms to complete at www.vitalrec.com which provide the required information you must submit. Call ahead to verify the amount you need to send. Currently the fee is $10.00 for birth certificates and $6.00 for death, marriage and divorce certificates.
