No. There are different forms for each plan. ·If you are a member of the Basic Health plan, you should use the ClaimSecure claim form which identifies group number 3100. ·If you have enrolled in the Voluntary Supplementary Health plan, you should use the ClaimSecure claim form which identifies group number 3101. These forms are available on the ACaeronet under the ‘My Retirement’ tab or on the ClaimSecure eProfile site. Claim forms are also available from Employee Services.
