Can my doctor fax a prescription to the Mail Order Pharmacy Service?

Yes, you may have your doctor complete the Fax Prescription Order Form (PDF). It is necessary for you, as the member, to complete the member, patient and payment information prior to giving the form to your doctor for completion. It is important that you use the correct form since the incorrect form can result in delays or unfilled prescriptions.
