Can the health system disclose patient information to outside referring physicians and hospitals?

Patient information may be shared, e.g., discharge summary, history and physical, op report, etc., with referring physicians for patient care purposes. Likewise, information may be shared with referring hospitals when both facilities have a role in providing care to the patient.  more
