In Sept 2008, the tooth was already undergoing an irreversible pulpitis and was doomed to eventually die, no matter what was done to it. Your dentist was trying to see from that time on, if the tooth still had a chance to recover. Often nothing can be seen on an x-ray for a long time. Often the symptoms can be confusing, as there can be periods of remission when things seem to be improving. But Monday morning quarterbacking at this stage, shows the tooth had no chance to make it. Your dentist did no thing wrong in trying to follow things and treat the tooth conservatively.
