Do all signatures need to be obtained on the ICJ Form IA/VI before submitting a referral packet?

No. If juvenile and guardian have left the state, we can obtain those signatures from the receiving state. Upon the form being signed, the form will be returned to the sending JPO for the adjudicating Judge’s signature. Upon obtaining the judge’s signature, the sending JPO would forward, in triplicate, three (3) copies of the completed ICJ Form IA/VI to our office.
