If the guiding catheter is not well located in the ostium, or a guiding catheter with sideholes is used, i.c. adenosine is unreliable and increasing the dosage is not recommended (you do not know what proportion of the drug will be spilled in the aorta). In this case, i.v. infusion is strongly recommended. WARNING: An overdose of the hyperemic drug may induce heart block.
