Do minor students (under the age of 18) need parental consent to receive care?

Unless it is an emergency, care for acute medical problems such as an upper respiratory infection or a tuberculosis skin test requires a parental consent. You can download the consent form and have your parent sign in the appropriate space. If a minor needs medical attention for a sexually transmitted disease, contraception and/or urinary tract infection, a parent’s consent is NOT needed.
