Do Prize Winners have to sign an affidavit of eligibility and publicity release?
Answer 8. Yes. This must be signed before prizes of $500 or more will be awarded. If you won a prize valued at $500 or more, you will receive a winner’s letter, an affidavit of eligibility, and a publicity release in the mail within 6 weeks from the date your prize claim was received by the Redemption Center. Winners of $1000 or more will also receive a W-9 tax form to complete and return. Once you have returned the properly completed paperwork, and we have received and verified your documents, you will receive your prize check within 6 – 8 weeks.
Yes. This must be signed before $10,000 vacation prizes will be awarded. If you won a $10,000 vacation prize, you will receive a winners letter, an affidavit of eligibility, and a publicity release in the mail within 6 weeks from the date your prize claim was received by the Redemption Center. $10,000 Vacation winners will also receive a W-9 tax form to complete and return. Once you have returned the properly completed paperwork, and we have received and verified your documents, you will receive your $10,000 prize check within 6 8 weeks. Winners of $25 prizes do not have to sign an affidavit of eligibility. Once the prize claim has been verified, you will receive your prize promptly (within 6-8 weeks from the date you mail-in your game piece).
