In my Church, we believe that those that are not able to distinguish right from wrong are not held accountable for their actions. This doctrine is primarily applied to young children who have not yet gained the necessary awareness of the consequences of actions to be able to make informed decisions. It is not just to hold such a person to the same standard as one who can understand such things. This doctrine has also been applied to those mentally handicapped to the point at which they never become able to distinguish right from wrong. I would say that the same would apply to someone who has a sufficiently severe mental disorder that they cannot distinguish fantasy from reality. However, I would also say that God would be the ultimate judge as to who can and cannot tell the difference.
