Recipients must invest at least 50% of funds in the first three strategic directions: tobacco-free living, active living and healthy eating, and high impact evidence-based clinical and other preventive services. Additionally, state and county recipients must award at least 50% of the total grant funding to other agencies and organizations, which can include other county or statewide partners, county or city health departments or other state or local governmental agencies, and or nongovernmental organizations to ensure local participation, support and effective implementation and sustainability of the program throughout the area. Rural areas of a state must receive at least 20% of the total grant award or an amount consistent with their proportion of the state population, whichever is higher. This 20% contributes to the overall 50% requirement for dollars to be sub awarded out to extend the reach of the program.
