The ADA has developed the 2006 version of the paper claim to accommodate NPI. It is available for use and should be considered the appropriate ADA claim form to use. When completing the paper claim, place the billing dentist’s or organization’s NPI is Block 49. Block 54 is specific for reporting the treating dentist’s NPI. (Please note that for sole proprietors and individuals, the NPI in both 49 and 54 will be the same). For electronically submitted claims (837D), please consult with your practice management software vendor to be sure that your NPI is correctly placed.
