To request that your criminal history review report receive official authentication*, you must complete and return the ACJIC Requests to Review, Appeal, or Challenge Criminal History Form and also include a completed Alabama Secretary of State’s Authentication Submittal Form. • The ACJIC Requests to Review, Appeal, or Challenge Criminal History Form and directions/requirements are available online at http://www.acjic.alabama.gov/documents/form_criminalrecords.pdf.
