How are changes made to the information in the TWIA agent database?

To change information on the TWIA database, the agent/agency must submit a request in writing, preferably on agency letterhead and signed by the agent or an officer of the agency. It is helpful if the agent/agency license number is provided. Please send to P.O. Box address below or use our Contact Page. Note: Name and address changes must also be made to the agent/agency license on file with the Texas Department of Insurance (TDI). Depending on the change, TWIA may request a current copy of the agent/agency license and a new W9 form.
