Compounded prescriptions are subject to your pharmacy plan’s Tier 3 copayment. Because compounding pharmacies can be difficult to find, you’re welcome to contact PacificSource Pharmacy Services at pharmacy@pacificsource.com or (800) 624-6052, ext. 3784 for help finding one that can prepare your medication. And even if you use a nonparticipating compounding pharmacy, you’ll still receive your plan’s Tier 3 benefits. You’ll need to pay for your compounded prescription when it’s filled and send your receipts to us for reimbursement, however. You may mail your reimbursement request to us at PacificSource, Attn: Pharmacy Claims, PO Box 7068, Eugene, OR 97401, or fax it to (541) 225-3665.
