• Within three working days the PHN contacts the family, childcare provider, and/or foster parents after receipt of notice from the coroner. • Contact may be a face-to-face visit, group visit, or telephone call. • Provides support services: • Assessment of the family, childcare provider, or both. • Crisis intervention and counseling. • Referral to community services. • Follow-up care to assess the progress of the family, childcare provider, or both. • Consults with the infant’s physician of record. • Submits documentation of services to the California SIDS Program.
