How do I acquire a copy of an emergency medical service patient report?
Come to our office during business hours with the date of service and location of call. Per HIPAA Rules and Regulations, patient reports will not be released without written authorization from patient themselves. There is a minimum charge of $2.00 for up to five pages and 50 cents for each additional page.
