You are required to complete a ‘Change of Details of an Operator Licence’ (P614A) form available from our website www.police.nsw.gov.au/sir or by requesting a form from the Customer Service Centre on 1300 362 001. You must send payment of $65.00 with your application to the SIR. A ‘Photograph Advice’ will be sent to you to take to the RTA to obtain your replacement licence.
