Contact the DSHS/DDD office at 206-568-5700 or 1-800-314-3296 and ask for an application. Information will be taken over the phone and you will be asked to provide any available school or medical records that will assist in determining eligibility. Copies of other documents may be required and be aware that eligibility cannot be determined until all of the necessary information is available Download DSHS/DDD forms here (external). If you are a family with a child under three, application for DSHS/DDD eligibility determination for your child will occur as part of the Early Intervention Intake and Eligibility Process.
