Section 5 of the “Application for Competency Evaluation” (exit DHS; PDF, 81 KB) must be completed by newly trained nurse aide candidates only. Only candidates applying under Eligibility Route E-1 must complete Section 5. If you recently completed a nurse aide training program, your nurse aide training program primary instructor will assist you and complete section 5 of the application form.
