You may request a new Eye Benefits discount vision card by: • Calling the Eye Benefits Customer Care Line at 1-800-621-7900 ext 4, OR • Visiting the Eye Benefits website http://www.eyebenefits.com. (this option is only available to actively employed subscribers with registered UVA email addresses). Click on “My Account” at the bottom of the homepage sign on with the username and password sent to you from Eye Benefits.
