The Mail Order Service Program provides the convenience of ordering maintenance drugs through the mail. Maintenance drugs are for people who require prescription drugs on a long-term basis. With this program, you may buy up to a 90-day supply of insulin and other covered prescription drugs through the mail. Your copay includes the costs of home delivery. Ask your Physician to prescribe needed medications for a 90-day supply, plus refills. If you are presently taking medications, ask your Physician for a new prescription. Ask your Employer for a member profile form. Complete this form with your first order only. Be sure to answer all the questions for yourself and your Dependents. Make certain you include your ID card number on the form. Send the completed member profile form, your original prescription(s) and the copay for each prescription in a pre-addressed order envelope.
