When you have selected your item, go to the checkout and opt to pay by credit card (not paypal). Make sure your browser allows ‘popups’ for this site. As you complete your checkout, watch on the left hand side of the screen, towards the bottom for an offer to take a ‘short survey to get a $25 CTC gift card’. Take that link as that is the link to get you into the Lifestyle Rewards program. You have to re-enter some information, such as your credit card # and address – and if all goes according to plan, you should get your Lifestyles membership number on the screen.
