How do I make arrangements to receive DMSO treatments?
The process begins with filling out the secure online form from our home page. Follow up by reading, signing and faxing an Informed Consent Waiver/Records Release to us at 918-493-6589. The form may be found by going back to our home page and clicking on “Consent Waiver.” When we receive your faxed form, we will schedule an appointment for you with our supervising physician. As soon as she reviews your case history and determines that you are a good candidate, your treatment can be authorized and we can then proceed. We ask that you bring a copy of your medical records with you to your first appointment.
