You can obtain copies of payments made to the Illinois State Disbursement Unit on your FIPS / Docket by calling Customer Service at 877-225-7077. On the Automated Voice Response System, you can request up to five of the last payments received within the previous twelve months be faxed to you, or you can speak with a Customer Service Representative to have your whole disbursement history mailed or faxed to you.
