AndrĂ© Goodfriend The Embassy does not keep copies of any Consular Report of Birth (CRBA) on file, but you can obtain an official replacement copy by sending a written request to the following address: Vital Records Section Passport Services 1111 19th Street, NW, Suite 510 Washington, D.C. 20522-1705 The written request for a replacement CRBA (FS-240) must be notarized (a UK notary is acceptable for this) and must include a copy of valid photo identification of the requester. The request must include: full name of child at birth (plus any adoptive names); date and place of birth of the child; names of the child’s parents; serial number, if known, from the upper right hand corner of the CRBA/FS-240 (on those issued after Nov.1, 1990); any available information from the applicant’s US passport (passport number, date of issuance, place of issuance, etc); signature of the requester.
