How do I obtain the submission form to submit a formalin-fixed tissue sample to the Diagnostic Center at MSU?
Diagnostic Center for Population and Animal Health necropsy biopsy submittal form should be completed by your veterinarian and shipped with the sample to the address found on this form. DCPAH is a separate facility from the Repository and the shipping address is not the same as the Repository. Please also complete the Repository submission form to notify us that your sample has been submitted for pathology. We will follow-up with you regarding obtaining a copy of this report for our records since the diagnosis is beneficial as it pertains to the samples donated to the Repository.
