How do I take a class Pass/Fail (note: this refers to classes that are normally graded, and does not apply to classes that have only a Pass/Fail grading basis)?
For undergraduates, written permission from the instructor is required. A Declaration of Undergraduate Pass/fail grading status change in a course form also needs to be signed by the student. Regulations for taking a course P/F are all explained on the form. It is located on T-Reqs on the Duke web site (see http://t-reqs.trinity.duke.edu/passfail.html). All requests need to be filed before Drop/Add ends.
