All requests must be filed before Drop/Add ends. For undergraduates, written permission from the instructor is required. Undergraduate students must also sign a Declaration of Satisfactory/Unsatisfactory Grading Basis in a Course form. Regulations for taking a course S/U are all explained on the form. It is located on T-Reqs on the Duke web site (see http://trinity.duke.edu/uploads/assets/S-U Grading Basis Form.pdf).
