How does assessment for transition care differ from assessing for pilots under the Innovative Pool, i.e. Intermittent Care Services (ICSs) and Innovative Care Rehabilitation Services (ICRSs)?
There are several differences in the eligibility criteria for transition care and the ICS and ICRS pilots. Two key differences are listed below. (Please note that the ICRS pilot program has concluded and that the ICS pilot program is due to conclude in June 2006.) The client eligibility criteria for transition care differ to those of the ICS pilots in that transition care only targets older people at the completion of a hospital stay, and not those older people who require additional support in the community to avoid entry to hospital. In addition, for transition care a person must otherwise be eligible for at least low level residential care in order to be approved, whereas for the ICS and ICRS pilots a person needed to be approved for residential care (or community care in the case of the ICS) in order to be eligible.
