0

ColonCancerCheck collects your contact information from the Ministry of Health and Long-Term Care (the “Ministry”) and your colorectal health and colorectal cancer screening information from laboratories and cancer planning departments within Cancer Care Ontario (CCO). CCO cancer planning departments may collect your personal health information for the planning and management of the health care system. These departments operate under CCO’s authority as a “prescribed entity” under section 45(1) of the Personal Health Information Protection Act, 2004 (PHIPA) – the health privacy legislation in Ontario.
