How does National Telemedicine Center obtain the enrollees medical history information?
In the initial registration procedure, members are asked to fill out a Medical History Disclosure form that acts as an electronic record of the enrollee’s medical history and health care profile. The disclosure form is similar to the information sheet a patient answers during his or her first visit to a PCP. It includes a thorough run through of past medical history, previous surgeries, chronic illnesses, cholesterol levels, medications, and allergies.
