A.The application for supervision is on our web site at www.tmb.state.tx.us/professionals/ncrt/ncrt.php. You can also email verifcic@tmb.state.tx.us or call (512) 305-7030 to request an application. A separate form must be completed for each non-certified radiologic technician supervised.
