How does the formulary work in a 3-tier copay?
One of the ways PHS maintains coverage of quality, cost-effective medications for you is through our 3-tier copay prescription drug benefit. The 3-tier benefit gives you control of how much you save on prescription drugs. You pay the lowest copay on generic drugs, while brand name drugs on the Preferred Health Systems Formulary require a higher copay. Brand name drugs not on the formulary require the highest copay. Actual copay amounts will vary based on the plan selected. Preferred Health Systems encourages members to request generic drugs whenever possible.
