Monthly physician visits, or three physician visits during a six-month nutritionist-led intervention would be sufficient for coverage purposes. Example: If a patient's weight loss attempt is being monitored by the primary care physician there should be no less than monthly visits with documentation for six consecutive months. If a patient's weight loss attempt is nutritionist-led, such as Weight Watchers, three physician visits during a six-month interval is sufficient along with the documentation from the Weight Watcher's weekly visits.  more
