How is the count of persons expressing a desire to return to the community determined?
Only residents whose admission is for long term care are considered. That is, residents admitted to a facility for convalescent care (e.g., a Medicare stay for less than 60 days) are not counted. Each resident’s most recent MDS assessment with a date no older than 120 days than the date the report is created is counted in the report.
