How long does a person need to be in hospital before they can access transition care?
There is no limit to the length of hospital stay prior to admission to transition care. In assessing an older person for entry into the Transition Care Program, the ACAT must ascertain that the person has completed their episode of acute care and any necessary subacute care, is medically stable and ready for discharge, and would benefit from transition care. This assessment must take place while the person is in hospital.
