A physician/practitioner who has established a corporation must obtain an NPI for himself/herself and an NPI for the corporation. A corporation applies for an NPI as an Entity type 2 (Organization) and the physician/non-physician practitioner applies for an NPI as an Entity type 1 (Individual). Report the Individual Entity Type 1 NPI in Section 1A where it indicates you are reassigning all your Medicare benefits and report the Organization Type 2 NPI in Section 4A and Section 4C.
