How should dollar amounts be submitted in Item 24f of the CMS-1500 claim form on paper claims?
Item 24f should contain the charge for each service listed with no punctuation. Both dollars and cents must be included (when applicable) with no spaces between them and with the cents portion to the right of the dotted line in this Item; dollar amounts to the left of the dotted line in this item. For example, $24.75 would be entered in 24f as 2475, and $.52 would be entered in 24f as 52.
