These are pretty typical problems for alot of people just starting out on barre chords, because for most people it will take time to build up enough finger strength to cleanly form and move these chords. About the only advice I have is that you should try to keep your wrist mostly straight, at about a 90 degree angle from the back of the neck, so that your arm is really coming straight back from the neck, instead of hanging loosely at your side. This will help you to get your index finger flat down across all of the strings, as you’re better off that way than trying to press down at an angle with the side of your finger. And, yeah, sliding is just going to take quite a bit of practice to be able to pull it off cleanly. There’s really nothing you need to change about the way you’re forming the chords, just slide the whole thing across the neck, keeping firm pressure on the strings.
