You may obtain the vision or limb waiver packet at any Texas Driver License office. 37 Texas Administrative Code 16.9, Texas Transportation Code 522.0235 or download the vision waiver packet or limb waiver packet. Note: Only original vision or limb waiver documents are accepted.
