I blacklisted somebody, but ORF says the sender is whitelisted by the Auto Sender Whitelist and allows the email in. How can I prevent that?

If you use a Private Local Database, you should simply add the sender to the exception list of the Auto Sender Whitelist database, so it will not be whitelisted anymore (ORF Administration Tool: Configuration / Tests / Auto Sender Whitelist / Settings / Check exceptions) In pre-4.2 versions, you can edit the private local database file contents using the Auto Sender Whitelist Browser (note that in 4.0-4.2 versions the ORF Service should be stopped first in order to edit the database). If you use an SQL database, you can delete the record using any tool of your choice (e.g.
