Beginning October 4,2004, vendors may no longer submit the Transaction Fee Report Form via fax or U.S. Mail. All vendors are required to submit the Transaction Fee Report Form via the online Transaction Fee Reporting System. Vendors, who have elected to do so, will receive an email notifying them when invoices have been generated for fee-eligible transactions.
