Log back on to the Health Risk Questionnaire site with your user ID and password and see your information stored to ensure it was properly recorded. Click on the Health Risk Questionnaire drop down menu in the top navigation for Questionnaire Confirmation. If you are uncertain, it is a good idea to log on before the deadline to be sure.
