Yes, the employee can add the spouse/qualified domestic partner to his or her coverage, if the employee completes an Employee Enrollment/Change (722.9 KB) form (HCA 50-400) and a Spouse or Qualified Domestic Partner Certification (706.2 KB) form (HCA 50-704) within 60 days from the date he or she arrives in the United States. Coverage is effective the first of the month following their arrival in the United States. The Employee Enrollment/Change form, Spouse or Qualified Domestic Partner Certification form, and a copy of the passport showing the date he or she arrived in the United States should be forwarded to the HCA.
